Tadalafil is a PDE5 inhibitor. It blocks the phosphodiesterase type 5 enzyme, which promotes smooth muscle relaxation in the penile vasculature and increases blood flow in response to sexual stimulation. It does not work without sexual stimulation. What separates tadalafil from the shorter-acting members of its class is duration: it stays clinically active for up to 36 hours, which is why it is often described as removing the need to time a dose to a specific moment. Food does not meaningfully affect its absorption.

Potential side effects include headache, flushing, nasal congestion, dyspepsia (indigestion), back pain and muscle aches, and dizziness or lightheadedness.
